Our Philosophy

Our policy is to try to prevent you from running out of medicines.  Please let us know when you are checked in by a medical assistant if you will be needing refills on any of your medicines.   If you do run out in between appointments, please try to give us three business days in which to address the refill request.


Our office works best when we can anticipate the number and type of visits we'll have per day, but we always keep a few visits open daily for "sick visits."  If you are sick, please call us to see if we have a same-day or next-day appointment for you.

First Time Visit

First Time Visit:
Ideally, on your first visit, we will have a "Meet & Greet" visit, in which we'll discuss your current and past health, review your family's medical history, determine if updated labs are needed, and order relevant past records to be sent. Of course, if you are ill on that day, we may just address your current issue and save the background for another visit.

We have a small lab where tests for urinary infections and strep throat can be performed while you wait. If your provider determines you need labs done on the day of an appointment, we will do it for you here, while non-urgent lab work is performed at the SED or TriCore labs. We will call you with abnormal results, and review normal ones at your next appointment.

HIPAA (Health Insurance Portability and Accountability Act)

The Health Insurance Portability & Accountability Act of 1996 (HIPAA) requires all health care records and other individually identifiable health information used or disclosed to us in any form, whether electronically, on paper, or orally, be kept confidential.  This federal law gives you, the patient, significant new rights to understand and control how your health information is used.  HIPAA provides penalties for covered entities that misuse personal health information.  As required by HIPAA, we have prepared this explanation of how we are required to maintain the privacy of your health information and how we may use and disclose your health information.


We are required by law to maintain the privacy of your health information. We are also required to give you this Notice about our privacy practices, our legal duties, and your rights concerning your health information.  We must follow the privacy practices that are described in this Notice while it is in effect.  This Notice takes effect 4/1/03, and will remain in effect until we replace it.

We reserve the right to change our privacy practices and terms of this Notice at any time, and to make the new Notice provisions effective for all protected health information that we maintain.  Revisions to our Notice will be posted on the effective date and you may request a written copy of the Revised Notice from this office.

  • Treatment: We may use or disclose your health information to a physician or other healthcare provider providing treatment to you.

  • Payment:  We may use or disclose your health information to obtain payment for services we provide to you.

  • Healthcare Options: We may use or disclose your health information in connection with our healthcare operations.  Healthcare options include quality assessment, reviewing competence or qualifications of healthcare professionals, or training purposes.

Unless you request otherwise, we may use or disclose health information to a family member, friend, or other personal representative to the extent necessary to help with your healthcare or with payment for your healthcare.  In addition, we may use your confidential information to remind you of appointments by leaving messages at home and/or work. We will also use our professional judgment and our experience with common practice to make reasonable inferences of your best interest in allowing a person to pick up filled prescriptions, medical supplies, or other similar forms of health information.  Any other uses and disclosures will be made only with your written authorization.  You may revoke such authorization in writing and we are required to honor and abide by that written request. We may use or disclose your health information when we are required to do so by law.

You have certain rights in regards to your protected health information, which you can exercise by presenting a written request to our Privacy Officer at the contact information listed at the end of this notice.

  • Access: You have the right to look at or get copies of your health information, with limited exceptions.  You must make a written request to obtain access to your health information.  You may obtain a request form by using the contact information listed at the end of this notice.

  • Disclosure Accounting: You have the right to receive an accounting of disclosures of protected health information outside of treatment, payment and health care operations.

  • Restriction: You have the right to request that we place additional restrictions on our use or disclosure of your health information.  We are not required to agree to these additional restrictions, but if we do, we will abide by our agreement (except in an emergency).

  • Alternative Communication:  You have the right to request that we communicate with you about your health information by alternative means or alternative locations.

  • Amendment:  You have the right to request that we amend your health information. (Your request must be in writing, and it must explain why the information should be amended.) We may deny your request under certain circumstances.

  • Written Notice:  You have the right to obtain a paper copy of this notice upon request.

Our Philosophy:
Dr. Carr's office is in some ways an old-fashioned practice, emphasizing thorough, personalized primary care that embraces the whole person.  It is not unusual to come in for a cold and also end up discussing osteoporosis risks and when your next cholesterol check should be.  We place a high priority on offering accurate, responsive, empathetic care.  We are also very interested in helping people with prevention of health problems, including smoking cessation, diet and exercise counseling, and stress management.
